New home from hospital
This free service is for people living in the Borough of Enfield who are leaving hospital and require some extra help and support:
We can help with:
· light household tasks
· basic food preparation
· medication collection
· shopping for essential items
We cannot help with:
· personal care e.g. washing, toileting etc.
· form filling e.g. benefits etc.
· giving medication

We accept referrals from any health professional, relatives and friends of patients and self-referrals.
Process
Once a referral is accepted, our staff will arrange a home visit soon after discharge from hospital to discuss the care plan.
Before the service commences our Supervisor will normally arrange to visit you and undertake a risk assessment. This assessment will help identify safety issues such as lifting and health hazards in your home. If hazards are identified the Homecare Manager/Supervisor will discuss the necessary action required to reduce risks to you or the Personal Care Assistant.

Option 1
Following an initial 2 hour assessment on the first visit the patient will receive :
¨ Week 1 —a one hour support visit
¨ Weeks 2 & 3 two weekly one hour visits
¨ Follow up support telephone calls
¨ Review call after six weeks
Option 2
Following an initial 2 hour assessment on the first visit the patient will receive :
· Week 1—2 x one hour support visits
· Weeks 2 & 3 - three x one hour support visits
· Follow up support calls
· Review call after six weeks
